Adult mental health targeted case management (AMH-TCM) services help adults with serious and persistent mental illness (SPMI) gain access to needed medical, social, educational, vocational, and other necessary services as they relate to the recipient’s mental health needs. AMH-TCM services include developing a functional assessment and individual community support plan, referring and assisting the recipient in obtaining needed mental health and other services, ensuring coordination of services, and monitoring the delivery of services.
Case managers assess clients’ functioning and needs for services. Case Managers meet with clients monthly or more if needed to assess, coordinate, and assist the client in gaining access to services using a person-centered approach. AMH-TCM does not provide direct care services such as housekeeping, personal care or transportation.. When a client needs these services, the case manager will identify resources and make referrals to services that meet the client’s needs.
The role of the case manager is to:
- Assessing the person’s needs, goals and the impact of the mental illness, and utilizing the client’s strengths and progress
- Establining and updating goal-related plans of the ICSP with the client
- Referring and linking to resources, services, and formal and informal supports
- Coordinating with other service providers and natural supports the client identifies as being important to the client’s recovery process
- Monitoring the effectiveness of the plan and the services provided to the client
- Discussing the progress made toward goals and recovery with the client and clinical supervisor
Eligible recipients for AMH-TCM must:

- Have a serious or persistent mental illness (SPMI) Or have a severe emotional disturbance (SED)
- Have completed a diagnostic assessment within the last 3 years and have an AMH-TCM recommendation
