NorthStar currently offers a variety of housing services including Housing Stabilization & Transitional Services. These services can be provided in St. Louis, Carlton, Lake, and Pine counties. Housing Services can assist individuals with finding and sustaining long-term housing options to live independently.
This service is a new Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance abuse disorder, and seniors find and keep housing.

Transitional services assist with developing a housing transition plan and supporting the individual in applying for benefits, navigating the application process, performing housing assessments, identifying resources to help with home goods, purchasing necessary items and expenses, and is reasonable for a person to transition from an eligible setting to their own home or an integrated community supports setting.
Through our services, you will receive:
Housing consultation to determine eligibility for programs, develop person-centered plans and assessment outcomes, coordinate services, and assist in understanding tenants’ rights to privacy and appeal information.
Housing sustainability including housing support, crisis intervention/management, education and training on roles, rights and responsibilities of tenants, assistance with housing recertification, ongoing support, and advocacy for the client
